style(REVISIONS.org):Change format to be more diff-compatible

Break up revision list into two lists:

1. File Locations
2. Revision History

Note: A benefit of having individualized histories of each file is
that it allows a future "Revision History" section to be added to a
drawing. Also, updates to `REVISIONS.org` will be easier to merge.
